Position Overview

PBCN Health Services is hiring one CHD / ADI worker. The CHD is responsible to work with the health team to provide health programming, education and a variety of health services to community members of all ages. Three aspects addressed within each program are: administrative duties, client support activities and community awareness. Works under the direction of the specific program nurse and Nurse Coordinator. CHD’s do move between programs and assist each other according to assignment.

Responsibilities:

Administrative Duties

  • Developing monthly work plans in conjunction with team members
  • Complete monthly reports
  • Budget management & planning
  • Maintaining client lists
  • Pulling charts, booking appointments
  • Ensuring adequate program supplies

Client Support Activities

  • Organize, coordinate and provide health related educational activities
  • Coordinate professional visits pertaining to chronic clients
  • Facilitate education and care activities
  • Home Visits as signed
  • Non-nursing client assessments

Community Awareness

  • Organize, coordinate and provide health related educational & screening activities
  • Promote Healthy lifestyle choices.

Qualifications

  • Grade 12 or equivalent
  • Additional education related to health, or the completion of post-secondary education is an asset
  • Fluent in Cree and English
  • Must be able to maintain confidential information
  • Must have interest and ability to participate in recreational activities
  • Excellent communication and public speaking skills
  • Computer skills and knowledge
  • Must have a valid Class 5 driver’s license;
  • Current Satisfactory Criminal Record Clearance, Vulnerable Sector Check Driver’s Abstract and proof of full covid vaccination.

Salary & Benefits

Salary: As per Salary Grid (depending on Education & Experience)
