Position Overview and Responsibilities

Working under the direction of the Community Health Team Lead, the Chronic Disease Nurse (CDN) is a Registered Nurse, with knowledge and experience in chronic disease management, providing comprehensive case management for clients living with or at risk for chronic disease.

The Chronic Disease Nurse will:

• Work within the Chronic Disease Registry and the Saskatchewan CDM-QIP Program

• Practice within a framework of holistic client and family - centered care, evidence-based interventions, health promotion, and disease / injury prevention.

• Work closely with the Nurse Practitioner and/or the client ‘s physician in the development of clientspecific care plans and medication management.

• Use an integrated care approach to managing illness which includes screening, assessment, treatment, referral, monitoring and evaluation, and client / family education. Qualifications:

• Registered Nurse licensed with the College of Registered Nurses of Saskatchewan (CRNS)

• Bachelor’s Degree in nursing preferred, or a combination of Diploma in Nursing and experience.

• Current Basic Cardiac Life Support Level C

• Minimum of 5 years relevant nursing experience

• Previous work experience in Chronic Disease Management or Community Health nursing with First Nation people

• Current Satisfactory Criminal Record Clearance, Vulnerable Sector Check & driver’s abstract;

• Ability to speak Cree is an asset
