JOB TITLE: Jordan’s Principle Coordinator

Position Overview

Reporting directly to the Community Health Manager, the Jordan’s Principle Coordinator will work to help PBCN families with children who need to connect to services and resources in the area of health and education and will be expected to work in collaboration with existing community and outside agencies. The Jordan’s Principle Coordinator will work with the PBCN Health Management team to provide program support and coordination of initiatives.

General Responsibilities

The JP Coordinator is responsible for carrying out the duties of the Jordan’s Principle (JP) Program by:

• Providing case management services to all children under the Jordan’ Principle program.

• Prepare & submit proposals to the Jordan’s Principle Regional Focal Points for review.

• Managing funds provided through the program for services/supports approved for families.

• Monitor & track budgets & proposals submitted to the JP program.

• Identifying children with unmet needs & assist the families to secure access to needed services & supports .

• Building relationships with service providers, and federal or provincial programs and services.

• Ensuring children receive quality and culturally appropriate health and social services.

• Networking and making connections for families with children with special needs.

• Arranging transportation & attending appointments with families, if requested.

• Setting up cultural & other supports for families.

• Supporting data collection and analytical activities.

• Establish and implement a communication and support network.

• Create awareness of the Jordan’s Principle Program in the communities through presentations, pamphlets, etc.

• Research and inform staff on Best Practice models.

• Compile monthly and yearly statistical and activity reports as required.

Qualifications

• Post-secondary education in a related field and/or other extensive human services experience .

• Research skills & competent in the use of computers; including excel, power point, internet and word.

• Strong interpersonal, written and oral communication skills.

• Highly organized with ability to plan, prioritize and coordinate multiple projects.

• Demonstrated ability to work effectively with individuals, as well as facilitate group sessions.

• Experience in working with and/or for First Nations.

• Valid driver’s license & Submit a satisfactory police and working with vulnerable peoples record checks .

• Knowledge of PBCN communities & ability to speak/understand Cree are assets.
